Prescription Drug Pre-Authorization Forms
Some prescription drugs require pre-authorization from Anthem Blue Cross Pharmacy management. If your pharmacist advises you that pre-authorization is necessary, please select the appropriate form below and fax it to your doctor. Your doctor must then fax the completed form to Anthem Blue Cross. Authorization takes approximately 72 hours.
