The chart shows the copayments, deductibles and coinsurance paid by the member. ProtectPlus also provides out-of-network benefits. However, if you use out-of-network providers you should expect to pay a significantly greater portion of the eligible charges. ProtectPlus now also offers Protect HSA 2,850, Protect 3500, Protect HSA 2,500 and Protect HSA 1,500 - four High Deductible Healthcare Plans (HDHPs), which when paired with Health Savings Account (HSA), may help reduce the cost of your healthcare coverage. In addition to the ProtectPlus copay plans and HSA plans, you may also select one of the Anthem Blue Cross HMO plans.
