Firms with more than 15 employees participating in the ProtectPlus program are eligible for special rates. Please contact Banyan Administrators, LLC 877-480-7923 for a quote. In most cases, all that is needed is a copy of your firm´s most recent health insurance invoice, or you may complete and return an employee census form. You may also view or print rates by clicking on your firm size: Firms of 15-50 participants, firms of 51-99 participants and firms of 100+ participants. Firm size and applicable rates are determined by the total number of owners and employees participating in the ProtectPlus program (including the Anthem Blue Cross HMO plans).
